Applications

NOTICE: All applications are now being scanned for electronic storage. Please avoid using the following when sending documents to our office:

• Colored ink
• Any color of highlighting
• Staples

Applications for each area of research are available on the following pages:

Our applications are available in both rich text files (.rtf) and Adobe Portable Document Format (.pdf). You will find that both versions are very easy to download.

• Rich Text Files (.rtf) are very easy to download and can be be imported into most Microsoft Windows and Macintosh word processors. RTF documents allow you to type directly into the document.

  To download RTF documents:

  • Click on the desired application link
  • Save your document locally (ie: desktop, hard drive, etc.) immediately after opening - entries that are done while on-line will not be saved.
  • Click on the answer areas provided and simply start typing in your answer
  • After completing the form, send the original and appropriate copies to the Office for Research Protections, 201 Kern Building
  • Documents are not available for submission online at this time
• Adobe Portable Document Formats (.pdf) are easy to download but are not writable. PDF documents preserve the look of the original, but must be printed out before typing on. Documents are not available for submission online at this time. Click here to download the free Adobe PDF Reader program.
