About this Site

We hope you find this site very helpful and easy to use. Some things you may find useful are:

  • Each page that requires scrolling to an individual topic within a page has jump links at the beginning to provide a quicker search.
  • Our applications are available in both rich text files (.rtf) and Adobe Portable Document Format (.pdf). You will find that both versions are very easy to download.
    • Rich Text Files (.rtf) are very easy to download and can be be imported into most Microsoft Windows and Macintosh word processors. RTF documents allow you to type directly into the document.

      To download RTF documents:

      • Click on the desired application link
      • Save your document locally (ie: desktop, hard drive, etc.) immediately after opening - entries that are done while on-line will not be saved.
      • Double click on the lines provided simply start typing in your answer
      • After completing the form, send the original and appropriate copies to the Office for Research Protections, 201 Kern Building
      • Documents are not available for submission online at this time
    • Adobe Portable Document Formats (.pdf) are easy to download but are not writable. PDF documents preserve the look of the original, but must be printed out before typing on. Documents are not available for submission online at this time. Click here to download the free Adobe PDF Reader program.
  • There are online training seminars available for your reading and a quiz at the end of each seminar that you can complete at your convenience. The quiz results are displayed immediately after completion. The Animal Worker Questionnaire and Occupational Health Verification packet and form, which is also considered part of our training, are available online as well.
  • Hotlinks to other areas within our site are provided on every page so navigation is easy.
  • A site index is available to quickly and easily identify the location of a page.
