Checklist for Submission

All IACUC weekly submissions MUST be received in ORP by 5:00 pm on Wednesdays

To ENSURE PROMPT PROCESSING, please check the following:

• ____ Sign the IACUC Application.
• ____ Enclose completed original application.
• ____ Enclose one copy of all reference documents.
• ____ Enclose approval from or application for Institutional Biosafety Committee or University Isotopes Committee, if required (refer to the question on Hazardous Agents).
• ____ One copy of grant proposal per submission, not per copy of the application
