IACUC Protocol Requirements

Requirement A – IACUC Online Training – some applications require this training for listed personnel (see below)
Requirement B - Occupational Health Training – some applications require this training for listed personnel (see below)
Appendix C - USDA Pain Categories (RTF) – Appendix C is required to complete some applications (see below)
Animal/Biohazard Safety Protocol (RTF | DOC recommended for Mac users) - This form is required for protocols involving infecting animals with any viable organism or involving animals, caging, or bedding material contaminated with hazardous materials that will pose a risk to personnel caring for or working with the animals.
Standard Operating Procedures (SOPs) – Some protocols involve animal procedures for which the IACUC has developed a Standard Operating Procedure. PIs using these procedures may indicate that they will follow the specific SOP(s) rather than detailing the procedure in the application.
