      This attachment is meant only for use WITH the Application for the Use of Biohazardous Materials (i.e., when the research involves the use of human blood and blood products, human body fluids, and/or human tissues along with other biohazards). If you are using only human blood and blood products, human body fluids, and/or human tissues in your research, please use the Application for Research - Human Blood and Blood Products, Human Body Fluids, and/or Human Tissue.

  • To request modifications (including personnel additions) to Institutional Biosafety Committee (IBC) approved protocols, please send an e-mail describing the requested changes to ORP-Biosafety@rtto.psu.edu. Please be sure to include the IBC protocol number or protocol title in the e-mail.
