Keeping Research Study Information Secure!Keep paper and electronic records safe and secure throughout the research study. 1. Are you using an Id Number and code list to link the data with the individual participant? If so, be sure to put signed consent forms in a different locked drawer space than the linking list and the data. The ID Number / code should not appear on the consent form. If the linking list is stored electronically, make sure a password, known only by approved study personnel, is in place to access the linking list. Data and the linking list need to be kept in separate files. 2. Security is a priority!Locked file drawers, locked rooms, limited access to storage areas, and password protected files are all ways to insure security. Consult information technology experts to make sure data and electronic surveys are sent, completed, returned and stored securely. 3. Check the IRB application. If you're not sure what to do with some documents, like screening forms for ineligible participants, check your IRB application - your plans for those or other forms will be outlined there. Sometimes screening forms are saved for later use, sometimes they're destroyed - it all depends on what was submitted by you to the IRB and what was approved (check "Additional Information" emails you may have received following the submission of your IRB application for any revisions).
