      All recombinant DNA (rDNA) activities at Penn State require submission to the IBC. Some rDNA activities (those exempt from the NIH Guidelines for Research Involving Recombinant DNA Molecules) may be submitted via this Prescreening Questionnaire.  Others require submission of  the IBC's full application. Use the Prescreening Questionnaire to determine: (1) whether the activities involve recombinant DNA (rDNA) techniques and (2) the required method of submission.

      This attachment is meant only for use WITH the Application for the Use of Biohazardous Materials (i.e., when the research involves the use of human blood and blood products, human body fluids, and/or human tissues along with other biohazards). If you are using only human blood and blood products, human body fluids, and/or human tissues in your research, please use the Application for Research - Human Blood and Blood Products, Human Body Fluids, and/or Human Tissue.

    • Appendix 2: Animal/Biohazard Safety Protocol (RTF | DOC recommended for Mac users)

      This attachment is meant only for use WITH the Application for the Use of Biohazardous Materials. This form is required for protocols involving infecting animals with any viable organism or involving animals, caging, or bedding material contaminated with hazardous materials that will pose a risk to personnel caring for or working with the animals.  Principal Investigators must contact Dr. Jeffery Dodds, D.V.M. (814-865-1495 / jwd12@psu.edu) prior to completing this form.  This document must be prepared in consultation with Dr. Dodds.  It must also be approved by Dr. Dodds before it can be submitted to the IACUC for review.

    • Appendix 3: Attachment for the Use of Infectious Agents that Require Handling above Biosafety Level 1 (RTF | DOC recommended for Mac users)

      This attachment is meant only for use WITH the Application for the Use of Biohazardous Materials
