Document Actions


All IBC applications are available as rich text files (RTF) and document files (DOC). Rich Text Files (.rtf) are easy to download and can be imported into most Microsoft Windows word processors. The DOC files are recommended for Mac users. Both document formats allow for typing directly into the document.

UPDATE: Effective immediately, all submissions may be made with an electronic application and remitted via e-mail to the ORP at All submissions must be sent from the Principal Investigator's CAC account.

To download and submit an application:

  • Click on the link to the desired format of the appropriate application.

  • Save the document locally (i.e., desktop, hard drive, etc.) - applications that are completed while online will not be saved. Save your document  at routine intervals and when complete to prevent information loss.

  • Click within the space provided on application and type in the complete answers.

  • After completing and again locally saving the form, click on the ‘SEND TO THE ORP' link at the bottom of the application.

  • Use the blank e-mail that pops up to send the application to the ORP. The correct address ( is automatically entered into the e-mail. All submissions MUST be sent from the Principal Investigator's CAC account (i.e.,

  • Attach the application to the e-mail and click SEND.
