Project Close Out Form

The ORP has a new application available, "Project Close Out Report for Research Involving Human Participants". This application may be found on the ORP website: http://www.research.psu.edu/orp/areas/humans/applications/index.asp.

The intent of this application is to allow investigators to close out their project(s) earlier than the scheduled continuing review period, which is based on your approval expiration date. A series of questions are asked on the form in order to gather the appropriate information before the project is closed including project status, activity status, project information, etc.

This application may be especially useful for investigators (and their advisors) who plan to complete their research in less than one year. Completion of the "Close Out" form will prevent multiple renewal letters from being sent to the investigator.
