Opportunity to Participate in Survey Research! Researchers at Penn State Hershey Medical Center are looking for loved ones of individuals with chronic illnesses to complete a survey that assesses preparation for future medical decision-making.
Eligible participants are:
- 18 years or older
- Have a loved one with a chronic illness (such as diabetes, heart disease, kidney disease, cancer, lung disease, etc)
- Can complete a written survey in English
Participants will complete and return a paper survey via a pre-paid envelope.
Participants will receive $10.00
