Remove an Entity from a Disclosure

If you no longer have a financial interest in a previously disclosed entity, you can remove the entity from your disclosure.

1. Log in to COINS with your Penn State account email address and password. (College of Medicine and Penn State Health users: see this page for log in instructions)
2. On My Home page, click Edit on your Disclosure.

3. Review the important information on the Welcome to your Researcher Disclosure Form page.
4. Navigate to the Identify the Entities page by using the Jump To menu in the upper menu bar. (NOTE: The Identify the Entities page only appears if you have identified a Significant Financial Interest (SFI) on one of the preceding four “Identify SFI Categories” pages.)

5. Entities you have previously disclosed are displayed.

6. To remove an entity, hover over the Entity name and click the x icon on the right.

7. A message will appear asking you to confirm the deletion. Click OK.

8. Click Save.

9. If you need to make any additional edits, use the Jump To menu to navigate to the relevant pages in the disclosure.
10. Once all edits have been made, click Save and then Exit to return to your My Home Page.
11. On My Home page, click Submit.

12. On the Submit pop up window, review information provided and check all the boxes. Click OK. You will receive an email confirmation from COINS to verify your disclosure was successfully submitted.
